2008 Dues and Registration Form
$59.00 United States Swimming (USS) Registration Fee:
 
Gold - Mon. thru Fri.  $80.00/mo. or $210.00/quarterly
Silver  - Mon., Wed. & Fri.  $70.00/mo. or $190.00/quarterly
Bronze  - Tues. & Thurs.  $60.00/mo. or $165.00/quarterly
 
 Due the 1st of each month
 
RFST is a non-profit cooperative of parents, coaches and swimmers. We work on a 
tight budget, so please make sure to pay your dues by the 1st of each month. RFST cannot 
sponsor a non-team member in any USS affiliated swim meet. For medical reasons or
extreme circumstances, credits will be given for another swim period within the
current season. Refunds will not be issued.

Liability/Medical Release Form
 
If ______________________________ (swimmer's full name) is injured while 
participating in programs offered by RFST, (1) I, the parent or legal guardian 
of the above named swimmer, agree to waive any legal claim against USS and those 
associated with USS, Pacific Northwest Swimming (PNS) and RFST; (2) I give 
consent for RFST to provide medical/athletic training attention, transportation 
and emergency medical services as warranted.
 
If injured while traveling to or from programs offered by RFST via public, private
or any other means of conveyance, I agree to waive any legal claims against USS, 
PNS and RFST.
 
By signing this release, I swear that the above named swimmer is in good physical 
condition and that I am not aware of any disease or injury that would result in 
her/his being injured during any program participation.

RFST is not responsible for swimmers prior to or after scheduled swim practices.
Please DO NOT drop your swimmers at the pool more than 20 minutes prior to 
practice. Please pick up your swimmers NO MORE THAN 20 MINUTES after a practice.
